About Us

Your Advocate for Life-Affirming Care

Our Story

Founded in 2016 by Seyi Adeoye, BA, BSN, RN, MPH, Effraim Home Care exists to minister to those in our community with the optimistic, compassionate, and highly skilled care services that foster optimum health and wellbeing. In fact, the name itself, Effraim, is the Hebrew word for “being fruitful,” and it’s our mission to ensure that each person in our care is thriving and enjoying fruitfulness in every season of life. Contact us online or call us at (215) 826-7422 to learn more about our home care team, habilitation and residential services.

To that end, we adhere to a set of fundamental core values:

Respect

We understand that each person we serve is a unique individual, and we artfully adapt our care to each client instead of asking the client to adapt to us.

Compassion

We approach all of our work with clients, their loved ones, and our own coworkers and professional partners with humility, kindness, empathy, and sensitivity to the needs of others.

Excellence

We act as efficient and effective stewards of our time and resources so that we can offer the best services and outcomes to our clients. We know that time is a gift, and we use it carefully, wisely, and skillfully.

Loyalty

We are supportive of our clients, their loved ones, our coworkers, and our professional partners, and we go the extra mile to make sure that we have served them in an excellent manner in every encounter.

Purpose

We believe that our purpose in life is to make a positive difference in the lives of others and to support them in continuing to experience fruitfulness in their lives.

We are pleased to provide non-medical in-home senior care and habilitation services in Philadelphia, Montgomery, Lehigh, Bucks, Berks, Delaware, and Chester counties in Pennsylvania.

Our Mission

Our mission is to deliver excellence and positivity in every encounter as we provide solutions to enhance our clients’ wellness outcomes.

Our Vision

Our vision is to help older adults and those with disabilities to continue to live fruitful lives by delivering the highest-quality, most user-friendly in-home care, habilitation and residential living services with a positive outlook and a focus on producing the best client care outcomes.

Care Team

Seyi Effraim, BA, BSN, RN, MPH, has deep roots in aging care. A trailblazing nurse guided by principles, core values, and vision to provide accessible, quality healthcare in the community she serves, and with expertise in public health and preventative care, Adeoye educates on health issues to impact healthcare delivery and outcomes, and to improve community health and safety.

Born in the UK and raised by accomplished parents, she moved to the US as a teenager and worked for her father’s established medical devices company, which developed an innovative in-home device for the elderly – sparking her passion for helping older adults and planting the seed that would eventually lead to the founding of Effraim Home Care.

A former director of nursing for one of the largest providers of home health in Pennsylvania, Effraim utilized her comprehensive clinical experience and her passion to help the elderly to grow her leadership to other areas of healthcare, such as the start-up of hospice care. Effraim explains, "To me, Effraim, or fruitfulness, means that it doesn’t matter what age you are, what season of life you are in; you can still be fruitful, and enjoy your 'greenness' in every season."

Our Caregivers

Effraim Home Care currently employs 100 care professionals, including a nursing director, registered nurses, social workers, therapists, client care coordinators, home health aides, caregivers, and more, enabling us to meet a continuum of needs: both those encountered now and those yet to come.

We are especially selective in the hiring of our care staff as we continuously strive to provide the highest level of care. We extensively screen and background check each person who desires to join our team of home healthcare professionals, and supervise all care by qualified registered nurses. We conduct criminal background checks, as well as review Department of Motor Vehicle records, for all potential employees as part of a comprehensive review process.

For the additional peace of mind of the families we serve, we offer the unique opportunity for family members to interview our caregivers in order to select the best fit for their loved one; and, if a family member prefers to provide the care personally, we will hire and train that person so he or she is fully equipped with the skills needed to ensure the very best care possible.
